Verifying insurance is an important step in delivering an outstanding patient experience!

The last thing you want is a patient to show up at your practice for their scheduled appointment, only to find out they are not eligible for an exam through their insurance. Even worse, if they cancel their appointment, you're left with an empty slot that is virtually impossible to fill!

Step 1: Go to the Eyemed Portal

Step 2: Log in with your user ID and password

Step 3: Search for the member using one of the following options:

Option A: Member ID Search

Option B: SSN Search

Option C: Name Search

Option D: ZIP Code Search

Step 4: Select the patient’s name

Step 5: Select a location and click on “Check Eligibility”

Step 6: Verify if the patient is eligible for an exam and/or materials under “Service Eligibility,” then choose a provider and click “Request Service Authorization for Selected Providers”

Step 7: Scroll down and click on “Show Member Benefits”

Step 8: A full, printable summary of the patient’s eligible benefits will display

You can then print this out, or save it as a PDF.
